Perfil neuropsicológico, características neuropsiquiátricas y estado funcional en el covid prolongado (long-covid): un estudio de casos y controles

dc.description.abstractIntroducción: La infección por SARS-CoV-2 se ha asociado con secuelas prolongadas a nivel funcional, neuropsiquiátrico y neurocognitivo. Este estudio tiene como objetivo investigar las diferencias en el rendimiento neuropsicológico, neuropsiquiátrico y funcional entre individuos con y sin COVID-19 prolongado (Long-COVID). Métodos: Se llevó a cabo un estudio comparativo de tipo ex post facto que incluyó a 15 pacientes con COVID-19 prolongado confirmado/sospechado (>12 semanas) y 15 controles que no experimentaron secuelas prolongadas de COVID. Se aplicó un protocolo neuropsicológico integral, junto con medidas de funcionalidad y síntomas neuropsiquiátricos actuales. Resultados: No se encontraron divergencias neurocognitivas globales entre los grupos. Sin embargo, el grupo con COVID-19 de larga duración mostró un rendimiento inferior en el lenguaje (p=0.01) y una tendencia hacia funciones ejecutivas más pobres (p=0.07). A nivel neuropsiquiátrico/funcional, evidenciaron una mayor frecuencia de síntomas ansiosos, depresivos, de dolor persistente y fatiga (p<0.05), así como limitaciones en el autocuidado y las actividades diarias (p<0.001). Conclusión: Aunque no se observó un deterioro neuropsicológico generalizado, las diferencias focalizadas en el lenguaje y el estado neuropsiquiátrico/funcional negativo resaltan la necesidad de un manejo integral y un monitoreo cognitivo continuo en pacientes con secuelas leves a moderadas a largo plazo debido a la COVID-19. Se requieren más estudios con una mayor representatividad y medidas neurales objetivas.
dc.description.abstractengIntroduction: SARS-CoV-2 infection has been associated with prolonged functional, neuropsychiatric, and neurocognitive sequelae. This study aims to investigate differences in neuropsychological, neuropsychiatric, and functional performance between individuals with and without long COVID. Methods: A comparative ex post facto study was carried out that included 15 patients with confirmed/suspected prolonged COVID-19 (>12 weeks) and 15 controls who did not experience prolonged after-effects of COVID. A comprehensive neuropsychological protocol was applied, along with measurements of functionality and current neuropsychiatric symptoms. Results: No global neurocognitive divergences were found between groups. However, the long COVID group showed inferior performance in language (p=0.01) and a trend towards poorer executive functions (p=0.07). At the neuropsychiatric/functional level, they evidenced higher frequency of anxious, depressive and persistent pain and fatigue symptoms (p<0.05), as well as limitations in self-care and daily activities (p<0.001). Conclusion: Although no generalized neuropsychological deterioration was observed, the focal differences in language and negative neuropsychiatric/functional status highlight the need for comprehensive management and continuous cognitive monitoring in patients with mild moderate long-term sequelae due to COVID-19. Further studies with greater representativeness and objective neural measures are required.
